There are now 2 ways to submit your Recertification Journal/Application.  

  1. Uploading to   **** Starting January 1, 2019, recertification will ONLY be handled through the website. *******
  2. Sending to the Recertification Chair person outlined below.

Sending to the Recertification Chair

As previously done in the past, you may still send your Recertification Journal/Application to the Recertification Chair.

The Recertification Chair is not responsible for any lost applications/journals. Please keep a copy of your application for your records. 

If paying by credit card, your recertification journal application with scanned copies of all backup documentation can be sent electronically to the email address below.

Please make sure your contact information is correct and the application is signed (lower left-hand corner.) If there are any issues with your application, you will be contacted by the Recertification Chair.
